How to get started with us

Step 1: Complete insurance verification form. All of our providers are in-network with Tricare and Medicare, there are a few other insurance carriers we are in-network with (United Health, Optum, VA Community Care), but it varies depending on the provider. Completing this form first will allow us to be up front with our potential patients about all costs and allows us to find the right provider. If we do not take your insurance after completing this form, our office will contact you. Or if you would like to self-pay, please move to the next step and complete the appropriate intake paperwork. If you plan to self-pay, please call our office because rates will depend on the service provided and the provider.

Step 2: Complete the correct intake form. If you or your child are seeking both therapy and medication management services, please complete the “psychiatric/medication management” paperwork only. Please note in the “presenting issues” section that both therapy and medication services are requested.

Step 3: Once we have your insurance or payment verified and the appropriate intake paperwork completed, one of our staff will call you to schedule your first appointment. We will not schedule an appointment with any potential patients until all the necessary paperwork is completed.
