Step 2: Complete insurance form. All of our providers are in-network with Tricare and Medicare, there are a few other insurance carriers we are in-network with, but it varies depending on the provider. Completing this form first will allow us to be up front with our potential patients about all costs. If we do not take your insurance, all of our providers accept cash, but rates will depend on the service provided and the provider. Call for rates. The insurance verification form can be found under the “Forms” tab.
Step 3: Once we have your insurance or payment verified, we will send the intake paperwork. Or these can also be found under the “Forms” tab. If you are interested in both therapy and medication, please complete only the medication paperwork.
Step 4: Once we receive your intake paperwork, one of our staff will call you to schedule your first appointment. We will not schedule an appointment with any potential patients until all the necessary paperwork.
